Physiotherapy Assessment: The First Step to Getting Better

I remember a patient named Anita who came to see me after months of putting it off. She had a nagging pain in her hip that made it hard to sleep on her left side. I told her that was not likely, but even if it was, we would cross that bridge when we came to it. The first step was just to figure out what was going on. That is what a physiotherapy assessment is. So What Actually Happens During an Assessment?

Let me walk you through it so you know what to expect. Your first visit starts with a conversation, a real one, not just a checklist of questions. We sit down together in a quiet room and you tell me your story. When did this pain start? What were you doing? What makes it better? And what makes it worse? Have you had this problem before? What have you tried already? What are you worried about? I need to know not just the medical facts, but how this pain has changed your life. Can you still play with your kids? Can you do your job? Or can you sleep through the night? After we talk, I will do a physical exam. I might watch you walk, see how you bend, check your strength. I will use my hands to feel for tight muscles and stiff joints. None of this is a test. You cannot fail. It is just a way for me to understand what is happening inside your body. Here is something that surprises a lot of people. The place where you feel pain is not always the source of the problem. Your knee might hurt because your hip is weak. During your assessment, I will look at the whole picture. I will check how your joints work together, not just the one that hurts. What We Are Looking For

When I do a physical exam, I am looking for specific things. Range of motion, how far can that joint move before it stops or hurts? Strength, can you push against my hand the way you should? Flexibility, are your muscles long and supple or short and tight? Stability, can you balance on one leg without wobbling? Gait, the way you walk, is it smooth or do you limp? Palpation, when I press on certain spots, do they feel tender or tight? Each piece of information is a clue. By the end of the exam, I usually have a pretty good idea of what is going on. But I will not just keep that information to myself. I will explain it to you in plain language. No confusing medical jargon. Just honest talk about what I found and what it means.

The Difference Between Assessment and Treatment

Some people think the physiotherapy assessment is just the thing you have to get through before the real treatment starts. That is not quite right. The physiotherapy assessment is the foundation that everything else is built on. Without a good assessment, treatment is just guesswork. Would you let a carpenter build an addition on your house without measuring the foundation first? Of course not. The same principle applies here. We need to know what we are dealing with before we can decide how to help. This is why we never skip steps. This is why we take time to be thorough.
